Have you registered on HCR?
Health Connect Registry (HCR) is one way BC communities are working to connect people with a Family Doctor or Nurse Practitioner. Registering on HCR is the first step to finding a primary care provider in Surrey-North Delta (SND), but it isn’t the last step. Knowing what to look for after you’ve registered can help you avoid missing out on important actions you need to take.

What to Expect
When a spot becomes available with a Family Doctor or Nurse Practitioner and you have reached the top of the list, there are two different ways you may be contacted, depending on the process of the healthcare provider you are being connected with:
Actions to Take
- If you would prefer to stay on HCR to be matched with another provider when more capacity becomes available
- If you’ve had an address change
- If you’ve have already found a healthcare provider and no longer need to be on HCR
If You Are Unreachable
What to Expect: Our attachment team and/or the medical office staff will make every effort to contact you when a provider becomes available. After three attempts, if we have been unable to connect with you via email or phone, we will assume you are no longer looking for a primary care provider and will inactivate your HCR profile and contact the next person on the list.
