What is SND?
Surrey-North Delta, or SND, is the region where our PCN operates. This area is divided into 6 neighbourhoods in which Family Doctors and Nurse Practitioners work, and PCN services are offered. The map further down on this page shows where each neighbourhood is located.

PCN stands for Primary Care Network. PCNs exist across British Columbia as local networks of health care providers. These networks aim to provide more connected care, with patients and their families at the centre.

Primary Care Networks exist all over British Columbia, with the goal of improving access to primary care services and better connecting patients, Family Doctors, Nurse Practitioners and other health care service providers.
In Surrey-North Delta, you need a referral from your Family Doctor or Nurse Practitioner to access Primary Care Network (PCN) services.
The Surrey-North Delta PCN team is hard at work connecting patients and primary care providers through the Health Connect Registry. So if you are looking for a Family Doctor or Nurse Practitioner, register today.

- PCN Clinical Pharmacy
- PCN Obesity Clinic
- PCN CHANGE Program (for people with Metabolic Syndrome)
In addition to these services, the Surrey-North Delta PCN works closely with other health partners and community agencies to enhance health services that already exist in our community, including:
